Final Forms Login

Physical Form

Huntington County Community Schools utilizes FinalForms for all physicals.

Physical Forms must be completed and uploaded to FinalForms.
Hold on to the physical copy of the Physical Examination form for your records.
The school does not need a copy of the physical form.
Parents and athletes must complete their parts in Final Forms and upload their physicals to be eligible to participate in the official first practice for the sport they have selected.
Certain school programs also require students to have a physical on file to participate (example: C.N.A. Clinical Program).
